Partnership and Program Implementation

Later, NHN launched its Community Health Worker (CHW) Program in Sanfebagar Municipality, linking households with Bayalpata Hospital through home-based counseling, maternal and child health follow-up, chronic disease tracking, and referral support. This integrated model, connecting hospital-based services with community outreach was later expanded to five additional municipalities across Achham: Kamalbazar, Mellekh, Chaurpati, Bannigadhi Jayagadh, and Ramaroshan.
As the program grew, it demonstrated significant improvements in maternal and child health outcomes, reduction in care delays, and increased healthcare-seeking behavior. This early initiative also laid the groundwork for NHN’s later contributions to evidence-based community health systems, digital integration with Electronic Health Record system (HER), and Government-aligned public health strategies, responding to the evolving needs of Nepal’s health system and global priorities for primary healthcare strengthening.
